How to Get an International Certificate of Vaccination (ICV) for Covid 19 in the Philippines During ECQ, 2021

On August 6, 2021, I went to Bureau of Quarantine (BOQ) Satellite branch in SM Mall of Asia (MOA) for my appointment. I am set to collect my Certificate of Vaccination (ICV) for Covid 19 which I’ll use for my upcoming international travel. 

Though possession of this card is not yet mandatory for local and international travel, I decided to get one for peace of mind.

Before I share how I got mine, let’s define International Certificate of Vaccination or Prophylaxis (ICVP)?

International Certificate of Vaccination or Prophylaxis (ICVP) is an official vaccination report created by the World Health Organization.

It is famously known as the Yellow card or Carte Jaune as the color of this document is yellow. 

This is an internationally recognized certificate and possession of this card may be required for entry to countries with an increased health risk for travelers.

Where can I use this International Certificate of Vaccination Prophylaxis (ICVP) for Covid 19?

This card can be shown to prove that you have received the full dose of Covid-19 Vaccine. Some countries like Hong Kong does not honor vaccination cards issued local LGUs, hence having an ICVP is recommended.

How can I get my own Yellow Card?

In the Philippines, the Yellow card is issued by the Bureau of Quarantine (BOQ). The applicant needs to book an appointment online (Click here) and pay a processing fee of Php 300.00. The applicant is also charged a convenience fee which ranges from Php 70 – Php 100, depending on the mode of settlement. 

My Experience

I booked my application on July 22, 2021, 2 weeks before my appointment. There are more available slots at BOQ Main Office, but looking at accessibility, the travel would be much easier at BOQ MOA Satellite office. By the way, the government has not announced the implementation of ECQ when I did my booking.

After the announcement of ECQ on August 6, I called BOQ’s trunkline, but the call is not going through. I decided to call SM Mall of Asia’s contact to confirm if BOQ MOA satellite office will be in operation. The operator mentioned that the BOQ office itself does not have an office line, rather she gave me a BOQ personnel’s number. At the same time, I have contacted BOQ’s Booking Support on Facebook regarding rescheduling. Both BOQ contacts actively respond to my inquiries.

I arrived at Bureau of Quarantine (BOQ) Mall of Asia (MOA) Satellite Office at 10:00am and I got my Yellow Covid 19 Card after 10 minutes. There were no long queues and the processing itself is quick (Click here to know how the processing steps) . I can say that this is best experience I had with any government agencies, ever!

Step by Step Process on scheduling an appointment for International Certificate of Vaccination (ICV) for COVID 19 in the Philippines using BOQ Online Booking System

Step 1: Visit BOQ Online Booking System website (Click here) and select your preferred BOQ office location. You will be re-directed to the booking page of your preferred location.

Step 2: Click “Book an Appointment”. Read the Terms and Privacy Policy then click the “I Agree to the Terms & Privacy Policy of this Website” to proceed.

Step 3: Fill in your email address, click the box next to “I am not a robot” then press “Submit”.

Step 4: On the “Service Category” portion, choose “Certification”. Choose “International Certification of Vaccine for COVID-19”, then click “Next”

Step 5: Select an available date and time for your appointment, then click “Next”.

Step 6: Fill in necessary information on the “Client Info” section, the click “Next”.

Step 7: Fill in information regarding your Valid ID and COVID-19 Vaccination. Upload a copy of your Valid ID and Front and Back side of your proof of vaccination. Hit the “Upload” button to proceed.

Step 8: Ensure that all provided documents are uploaded successfully. If so, hit “Next”. If documents uploads fail, go ahead and click the “Re-upload” button.

Step 9: Choose your preferred method of payment. (Click here)

Step 10: Follow settlement instructions per your chosen mode of payment. Once payment is confirmed, you will receive a text message from PisoPay confirming your appointment.

How to Verify your Appointment Status

Step 1: Visit BOQ Online Booking System website (Click here) and select the location for your appointment.

Step 2: Click “View appointment status”

Step 3: Fill in your “Reference number” and “Date of Birth”, then click “Submit”. Wait for your appointment status to appear.

Step by Step Process on Processing International Certificate of Vaccination (ICV) for Covid 19 

Step 1: Screening 

Present Valid ID and Vaccination certificate or Vaccination card to BOQ personnel for verification.

Step 2: Releasing

BOQ personnel will pull your information from their system. BOQ personnel will then request you to go through the information for confirmation before processing the Yellow Card.

Step 3: Doctor on Duty

Proceed to the “Doctor on Duty” area and present Yellow Card to be signed by the doctor.

Step 4: Signature

Sign on the check mark on the bio page of the Yellow Card.

How to Verify BOQ Certificate

Step 1: Open BOQ website and click “Verify BOQ Certificate” button.

Step 2: Scan the presented QR code on chosen device. 

Step 3: After scanning, the details of the card holder will automatically pop-up.

How much does each certification cost?

Each certification costs PHP 300. For Senior Citizens, it costs PHP 240. An additional Php 70 – Php 100 is charged for convenience fee, depending on your chosen mode of payment.

Is it necessary to get an International Certificate of Vaccination?

Depending on which country you are traveling to, you might need an International Certificate of Vaccination to prove that you have received a full dose of  Covid vaccine. Countries like Hong Kong acknowledges ICVP for Covid 19 Vaccine more than certificates issued by respective LGUs. For good measure and for peace of mind, having ICVP for Covid Vaccine is recommended.

Can I use my vaccination card issued by our LGU for my BOQ booking?

Yes, you may use your LGU issued vaccination card to book for an appointment. You are required to scan the front and back page of your card to proceed with the booking.

Can I rebook my booking to a different date?

There is no option from the BOQ interface where you can re-book your appointment. However, you may send an email to helpdesk.clinic@quarantine.doh.gov.ph along with a screenshot of your original appointment and payment receipt for re-scheduling.

I do not want to carry on with my booking. Can I have my payment refunded?

All payment is non-refundable. Payment shall be forfeited for:

  • Applicants who fail to show up on their confirmed appointment
  • Applicants who cancels their appointment,
  • Applicant whose application was rejected due to inconsistency and/or incorrect information, and
  • Applications who present discrepant and/or fake spurious documents

Can I get my IVCP or Yellow Vaccine Covid 19 Card even though I got my complete covid shots abroad?

No. Yellow Card will not be issued to applicants whose vaccines were taken abroad. BOQ will have to validate the vaccine’s code number and they won’t be able to do that for vaccines issued abroad.

Can someone collect my ICVP on my behalf?

Only immediate family members can collect ICVP on behalf of the applicant. The representative has to present an original ID of the vaccinated individual, original ID of the representative, authorization letter and original copy of vaccination card of the applicant.

Can I go to other BOQ branch even though I booked my appointment on another branch

You may only go the BOQ branch where you did your booking. You cannot visit a different branch with the same booking reference. 

What should I bring with me for my appointment?

Please be sure to bring original copies of the required documents on your date of appointment.

    • ACR/ICR

Is there any dress code I should follow?

Applicants are expected to wear appropriate attire on their appointment date. No shorts, No open shoes/ slip-ons, No sleeveless and sandos are allowed

What is BOQ’s Operating times during ECQ?

BOQ MOA’s operating time during ECQ is from 8:00 AM – 2:00 PM

Will I receive an email confirmation that my appointment is confirmed?

Based on my experience, I have only received a text notification from PisoPay that my booking is confirmed.

Is it necessary to book for an appointment or BOQ braces welcome walk-in applicants?

Booking an appointment is mandatory. No walk-in applications will be entertained. 

What time should I go to BOQ branch?

It is advisable that you come exactly at your appointment time. Your appointment time and branch is validated before processing you application.
